The Canadian Memorial Chiropractic College (CMCC) is committed to protecting the privacy and confidentiality of personal and personal health information entrusted to it by patients attending the CMCC Clinics.

CMCC Clinics collect and retain personal and personal health information necessary for the provision of patient care and for related administrative purposes. This information may include, but is not limited to:

  • Gender and age

  • Health history and relevant personal health information

  • Examination findings and test results

  • Health conditions, diagnoses, and prognosis

  • Details of care provided, including treatment records

  • Discharge recommendations

  • Billing and payment information

 

Use and Disclosure of Information

Personal and personal health information collected by CMCC Clinics is used solely for purposes related to patient care, education, administration, and compliance with legal and professional obligations.

CMCC Clinics do not disclose personal or personal health information without patient consent, except where disclosure is permitted or required by law.

Patients have the right to:

  • Request corrections where appropriate

  • Ask questions about CMCC’s privacy practices
